Dr. Carly Polland, ND is a true partner in your health journey. She provides long appointments so the two of you can fully investigate your symptoms. She truly listens and does not dismiss any of your concerns. Her goal is to dig deep into your case to uncover the root causes so your symptoms won’t just improve, but actually resolve. She offers truly personalized care, never relying on stock protocols, and always collaborating with you to develop a plan aligned with your goals and values.

How It Works

Step 1: Trial Appointment

Start with a 15 minute appointment to make sure you and Dr. Carly Polland will be able to successfully achieve your health goals. This is not another sales pitch but an exploration of care. You will discuss what health topics you may explore during your care, what labs might be ordered, and generals about treatment approach, timeline and cost.

Step 2: Medical history

Prior to your first appointment, Dr. Carly will dig into your medical history through your past medical records and extensive health questionnaires. She will start connecting the dots to determine the root cause of your symptoms. While you wait for your first appointment, you can get started on your healing journey with Dr. Carly’s Foundations of Health, a series of videos and handouts on the basics of nutrition, stress, sleep, and health mindset.

Step 3: Initial Evaluation

The first appointment is a 60 minutes, reviewing Dr. Carly’s analysis of your medical history and symptoms. You will finalize lab orders and get started on a minimal treatment plan. Labs may include blood, stool, saliva, or urine samples. Blood samples can be collected at a local Quest or at your home with a mobile phlebotomist. Stool, saliva, and urine samples can be collected at home. Test results may take 2-14 days.

Cost: $475

Step 4: lab review

Once all your lab results are in, you will meet with Dr. Carly for your second appointment. She will break down your lab results so you have a clear understanding of what is causing your symptoms. You and Dr. Carly will then strategize a treatment plan that targets the root cause of your symptoms. Treatments may include nutrition, supplements, herbs, medications and lifestyle changes. Patients receive a 20% discount on physician-grade supplements.

Cost: $375

Step 5: Follow Up

Follow up appointments are generally every 2-4 months for 12 months (if needed). You may need to complete abbreviated labs prior to your follow up appointments to track your progress. The goal is to get you into health maintenance as quick as possible. Most patients reach maintenance after 9-18 months of care. Once in maintenance, patients can do annual labs to monitor their health.

  • Dr. Carly is an out-of-network provider. Your insurance coverage of her services depends on your out-of-network coverage.

    You may be able to use HSA or FSA to pay for appointments, labs, and possibly supplements.

    Not all services Dr. Carly provides will be covered. For example, some insurance companies will not reimburse for phone or extended visit codes.

    Dr. Carly will provide you with a superbill with the accurate codes once you have paid in full for that service.

    Lab coverage is a little different. It depends on whether the lab is in or out-of-network and it depends on whether or not your insurance company thinks the labs are medically necessary. Labs ordered at in-network labs (like Quest), if considered medically necessary, will be covered by your lab copay or applied to your in-network deductible. Out-of-network labs are generally applied to your out-of-network deductible. Talk to your insurance company for more information.

    How to Determine Insurance Reimbursement

    1. Call the customer service number on the back of your insurance card

    2. Ask if you have out-of-network coverage

    3. Find out if you have a deductible for out-of-network coverage and what percentage they will cover for out-of-network costs

    4. Find out if they will cover labs ordered by out-of-network providers, if you have to meet a deductible, and what percentage they will cover

    5. For labs, ask if they have a preferred out-of-network lab

    6. Ask how you can submit your claim, making sure to get the correct forms and mailing address (they may tell you to have the doctor submit the claim, but you need to, so be sure to get all the info you need)

    If they ask what a naturopathic doctor is, tell them a naturopathic doctor is a licensed primary care provider in the state of California

  • No. As long as you are somewhere in California or Washington,

  • No. Dr. Carly is not a primary care provider. For women’s health exams, sports physicals, vaccinations, and acute care, you will need to consult with another doctor.
